FP10HNC. FP10SS. Green. These FP10 Prescriptions may be issued by GPs or nurse and pharmacist prescribers, hospital doctors, supplementary prescribers or out of hours centres if correctly annotated. Forms annotated with the initials RD are repeat dispensing forms.

- Medical exemption certificate.
